Neurological illnesses are complex in nature and often require long-term care. Neuro patients may have to undergo several hospital admissions as they battle the illness. And discharge from the hospital doesn’t necessarily mean they don’t need medical support anymore. In fact, taking care of the patient becomes all the more important after discharge from the hospital.
Post-hospitalization care at home is often managed by family caregivers who are not medically trained. During this time when patients are vulnerable to adverse events, it is crucial to have trained medical support at home.
Ubiqare enables medical support to help patients and their families manage neurological conditions at home. Our home health care service can be used under different circumstances.
At times patients undergoing active treatment for neurological conditions are not required to stay in the hospital for extended period. But this doesn’t mean they don’t require medical attention. Such patients require care that supports them as they undergo the treatment. This may include helping them stabilize their blood pressure or blood sugar levels, administering medicines, treating side effects, etc.
Certain neurological patients require extended rehabilitation to help them regain normalcy of life. They end up visiting rehabilitation centers multiple times a week for several months. Instead, we enable rehabilitation care at home where patients are most comfortable. We arrange for specialists like physiotherapists, speech therapists, etc. to visit patients at home and work with them.
Certain neurological conditions may not be treatable or are diagnosed at an advanced stage where treatment is futile. In such cases, we enable palliative care to make patients as comfortable as possible at home. We help them deal with symptoms of their illness, offer pain relief, and help maintain quality of life as the illness progresses.
When designing our home health care service, we set the following as our guiding principles:
• The patient should have easy access to a doctor
• The patient’s condition should be monitored regularly
• Reduce avoidable hospital visits or readmissions
Keeping these in mind, we offer the following in our home healthcare service.
We assign Care Doctors to work with patients at home and implement the treating specialist’s care protocol. Our doctors coordinate with the treating specialist and share regular updates on the patient’s condition. They monitor the patient regularly and offer treatment for managing symptoms or side effects at home. They also offer emergency clinical assistance to patients if needed.
One of the advantages that patients and their kin believe they have at hospitals is that of regular patient monitoring. We are extending this advantage at home. Under our service, the patient is regularly monitored by a nurse and our Care Doctor. Their vitals and health complaints are digital recorded and shared with the treating specialist – similar to the hospital set up. Any concerning changes to the patient’s condition are immediately reported and specialist advice is sought and implemented.
Neurological conditions require long-term care which may be difficult for some to manage all by themselves. They may lack proper understanding of how to perform certain care tasks. They are often also not trained to handle certain clinical situations like administering of medicines, cleaning of feeding tubes, catheter, etc. We provide nurses and caretakers to help such families.
Our specialty nurses also perform supervised nursing interventions at home to manage clinical episodes at home which otherwise may require hospital visit. Such timely clinical support also minimizes the risk of worsening of complications.
Home is one place where chronic illness patients feel most comfortable. While hospitals do their best to treat neuro patients, the longer recovery period is often spent at home by patients. This is also the time when patients are most vulnerable to adverse events. Having a doctor-driven medical support system available at home can help reduce avoidable hospital readmissions and reduce the cost of care as well.
