We assign dedicated care doctor to each patient to monitor the condition and engage with family members to help them understand the situation. Our care doctor is available – online and in person – in times of need.
Extension of Specialist Care
If the patient is undergoing specialist treatment, we extend the specialist’s follow up care at home through our platform. We incorporate the specialist care protocol in our platform which is then implemented by our Care Doctor.
Supervised Interventions at Home
Our medically trained team provides skilled clinical interventions at home under doctor supervision. These interventions offer relief to the patient and help reduce avoidable hospital visits, readmissions, and complications.
Lab Testing Services at Home
Elder patients who are plagued with complex illnesses often require lab tests to be done at regular intervals under their health monitoring protocol. We coordinate these tests and provide sample collection services at home to reduce their lab visits.
Equipment & Medical Supplies
We make medical supplies like adult diapers, underpads, etc. available at home to elderly patients. In special cases, our Care Manager also helps arrange for things like wheelchair, walker, special bed, etc. from our vendor partners.
